Direct care practices are evolving. Many clinics today offer far more than a single monthly membership — they’re launching weight-loss programs, women’s health services, behavioral health offerings, chronic disease management programs, IV therapy, nutrition consults, and more in addition to their core membership offering.
Hint’s Product Team heard directly from hundreds of practices and more than half said they want to offer multiple memberships per patient, and nearly a third want to package or bundle services in ways that require more flexible billing logic.
Today, we’re excited to introduce Multiple Memberships, a new Hint feature that gives practices the freedom to enroll patients in more than one retail membership at a time — plus a sponsored membership — all within the same platform. This enhancement opens the door to richer care models, new recurring revenue streams, and a smoother operational experience for clinics and patients alike.
Why Multiple Memberships
Direct Primary Care (DPC) and membership-based care have matured rapidly. Practices are no longer defined by a single “core” plan — they’re expanding into multi-specialty arrangements and complementary services that patients are eager to participate in.
Prior to this release, we found practices were using hacks and work arounds to accomplish managing multiple memberships for patients, including:
- Using Google Forms or paper agreements
- Hacking plan names to represent plan combinations
- Managing custom billing calendars manually
- Tackling edge cases (like dependent enrollment) on spreadsheets
These workarounds were time-consuming, error-prone, and made it difficult to scale. Multiple Memberships eliminates these barriers, giving practices a clean, structured way to offer and bill layered care models.
Offer More Services Without More Systems
Now, clinics on Hint can build care offerings that better reflect the diversity of services patients want. A patient can join:
- A core DPC membership
- A 6-month weight-loss program
- A mental health add-on
- A specialty program like women’s health, men’s health, pediatrics, integrative medicine, or acupuncture
Each membership can have its own billing cycle, pricing, and eligibility rules - all managed in one place.
Additionally, practices can now enroll dependents in their own memberships, such as a child-only pediatric plan or a spouse-only weight-loss program. Family enrollment no longer has to follow a single pattern.
This flexibility makes it easier to reach new patient segments, including patients who may not initially be interested in a full DPC membership but are drawn to a specialty service.
Increase Revenue per Patient — Sustainably
Layered memberships don’t just improve patient choice — they expand a practice’s financial engine.
For example, if a patient pays $100/month for DPC over three years, their Lifetime Value (LTV) is $3,600. If the same patient enrolls in a $50/month weight-loss program, their LTV increases to $5,400 over the same period — a 50% boost.
This represents a major opportunity to:
- Develop new recurring revenue streams
- Increase transaction volume per patient
- Improve long-term financial sustainability
- Create resilience against patient churn
Multiple Memberships gives clinics a stable, structured way to offer additional services without piecing together external billing tools or fragmented systems.
Support Sponsored Patients Who Want More
Many clinics serve patients through sponsored plans — typically employer-paid memberships. Historically, these patients couldn’t easily enroll in additional self-pay memberships without administrative complexity.
Now, sponsored patients can join any number of retail memberships alongside their core sponsored plan. This unlocks new clinical pathways for employers and gives patients more control over their care.

Simplifying Billing, Agreements, and Admin Work
Multiple Memberships includes key updates to make multi-plan billing clean and predictable:
- One invoice per membership
- Flexible billing cycles and charge timing per plan
- Separate agreements stored per membership
- Plan-aware coverage rules, including smart “bargain hunting” if multiple memberships cover the same charge
- Clear visibility into all memberships in both Provider and Patient portals
- And coming soon: the ability to set different default payment methods per plan
These improvements streamline and automate away the work to support multiple subscriptions. Everything now lives in one place, with full billing clarity.
Designed to Work Across Your Clinical Ecosystem
Clinics no longer need to track complex membership structures across disparate tools. To support modern workflows, your different membership types integrate seamlessly across Hint Clinical and Elation. The updated Memberships Report also provides a unified view of all memberships across retail and sponsored plans
Built in Collaboration with Practices
This feature was informed directly by customers as one of the top feature requests. In our discovery with ~100 practices:
- 50%+ want to offer multiple memberships
- ~30% want multiple memberships and bundled offerings
- Many are already selling specialty programs — but outside of Hint
Multiple Memberships gives practices a modern way to scale these offerings, experiment with new ideas, and deliver the comprehensive care models they envision.
A More Flexible Future for Direct Care
Direct care is growing — not just in size, but in possibility. Practices are finding new ways to serve patients, build sustainable revenue, and operate more efficiently. Multiple Memberships reflects that evolution, giving clinics the tools they need to innovate without operational constraints.
We’re excited to see what you build with it — from specialty programs to multi-service bundles to new models we haven’t imagined yet.
More offerings. More flexibility. More ways to care.
